Fat dissolving injection, medically called injection lipolysis, is an effective treatment against excess fat deposits. An active ingredient is injected under the skin that biochemically breaks down and dissolves fat cells. The process is similar to natural fat burning in the human body. It is low-risk and without major side effects.

The treatment with fat dissolving injection is suitable for well-defined problem areas such as double chin, neck or upper arms. It is no cure for obesity and does not replace a diet or a healthy lifestyle. The treatment must also be repeated several times at intervals of 4 to 8 weeks to provide best results. Patients who want to get rid of their double chin or lose belly fat, but are otherwise satisfied with their appearance, have long faced a dilemma: Losing fat specifically in certain areas of the body is not possible. It is true that you can force yourself into an energy deficit through diet and exercise, and thus burn more fat in general. But which fat deposits the body taps into and metabolizes cannot be planned. Whether you’re more likely to lose belly fat, get rid of that excess double chin, or tone up thick upper arms is beyond control. However, if you want to lose fat in a very specific area, until a few years ago, your only option was liposuction.

Lipolysis against excess fat deposits

Therefore, alternatives have been researched for a long time. And finally found with the fat dissolving injection. Its active ingredient, derived from the soybean, can burn local fat deposits with precision, down to the centimetre. And this naturally and without great risks. Moreover, in comparatively inexpensive fashion. The fat loss that the fat dissolving injection triggers is similar to the process that takes place in the human body during fat burning. This is one of the reasons why there have never been any serious incidents since the invention and early applications of the fat dissolving injection.

Losing Fat naturally requires patience

Because the fat dissolving injection mimics the body’s natural fat-burning process, it does not achieve dramatic overnight results. After the first treatment, it takes a good 2–4 weeks for the first signs of fat loss to become noticeable. And for clearly visible results, the treatment must be repeated several times, usually 3–4 times in an 8-week interval. This may be seen as a disadvantage of the treatment. Nevertheless, it will be understood that a natural fat loss just takes its time. After all, one is spared a risky and typically costly surgical procedure. For normal-weight people in particular, the fat dissolving injection represents an interesting option for losing excess fat in problem areas that cannot be addressed with exercise and diet.

Who should not get a fat dissolving injection?

Firstly, injection lipolysis is not intended to reduce obesity. For this, a suitable diet is the more appropriate means. Moreover, it is not an efficient means in such cases where the goal is to remove large fat deposits without dieting, typically on the abdomen, hips, and thighs. If the quick way is sought here, then liposuction is a better alternative.

Moreover, pregnant women and nursing mothers as well as diabetics with vascular diseases (microangiopathies) must not be treated with fat dissolving injections (absolute contraindication). The treatment is relatively contraindicated in cases of autoimmune diseases (e.g. scleroderma), severe liver or kidney disease, allergies to one of the ingredients or blood clotting disorders. In addition, in acute infections, severe obesity (BMI > 30).

Fat dissolving injection in 5 steps

tHE Process

In my practice in Munich, the treatment with the fat dissolving injection follows a fixed five-step process. Since the treatment requires a topical anesthesia with lidocaine cream, and this requires about 30 minutes of exposure time to reach full effect, it takes a relatively long time all in all. But since patient comfort is important to me, I take the time necessary to carry it out properly.


The first step in the process is a detailed discussion of the patient’s current situation and aesthetic goals. What should be achieved with the fat dissolving injection? Is the goal realistically achievable? What are the costs? What are the risks? Are there alternative methods that would be more suitable or cost-effective? Are there any contraindications that need to be considered? If all questions are answered satisfactorily, then the patient signs a consent to treatment and a fee agreement. This bindingly records the costs of the treatment. Subsequent “surprises” are not possible. This step takes 15–30 minutes for first-time patients. It won’t take nearly as long in subsequent treatments. The local anaesthetic can be dispensed with if the treatment area is small and only a few punctures are required.

Cleaning and disinfection

Thorough cleaning and disinfection of the treatment area are essential to exclude bacterial infections. Infections with subsequent scarring, e.g. abscesses, are one of the principal risks of any treatment using injections. To keep this as low as possible, traces of make-up and other impurities must be completely removed. I then disinfect the treatment area with a skin-friendly disinfectant. This step takes 5 minutes.

Local anesthesia

Since the fat dissolving injection is administered in quite many punctures, local anaesthesia is highly recommended. I usually numb with a lidocaine cream. It requires a contact time of 30 minutes to take full effect.


Before the actual injections, the treatment area is first cleaned of the lidocaine cream. The injections with the active ingredient are relatively quick and typically take no longer than 15 min. The needles I use for this are very thin. For larger treatment areas, I use a special multi-injector with which five injections can be administered at the same time, if this is tolerable for the patient.

Aftercare and control

After the injection follows a look in the mirror, a short check if everything is okay and the result meets the expectations – and we are done! Due to the large number of punctures, your skin will be very red. Haematomas are also possible. The traces of the treatment may remain clearly visible for 1–3 days. You should take this into account in your scheduling of appointments and events. Immediately after the treatment, there will also be a strong swelling in the treatment area, often extending into neighbouring zones. This swelling usually lasts for 1–3 days. It typically occurs with a distinct burning sensation and pressure sensitivity that also can last for several days. These side effects are normal. They should have subsided by the time of the follow-up appointment in 14 days. If they are visible or noticeable for more than 1 week, you should contact me immediately.

How does the fat dissolving injection work?

Nowadays, there are different variants of the fat dissolving injection, which are based on different active substances or combinations of active substances. The variant I use is the so-called “network compound” of the lipolysis expert group in the Network Global Health, an association of medical professionals. It consists of a combination of the two active ingredients phosphatidyl choline (PPC) and deoxycholic acid (DOC) and thus goes back to the well-known drug Lipostabil. Lipostabil was developed for the prevention and therapy of fat embolisms and has been in practical use for over 30 years now. It contained PPC, a highly purified lecithin, and DOC, an aggressive bile acid. In the treatment with the fat dissolving injection, the latter has the task of destroying the membranes of the fat cells. The PPC then supports the metabolizing of the exposed fat and its transport to the liver.

With the drug Kybella (Allergan) and the medical product Aqualyx (Marllor Biomedical), alternative formulations have been brought onto the market a few years ago. Both rely on DOC as the only active component. In my opinion, this does not change the efficacy and side effect profile much, and the treatment protocols are also similar: all preparations achieve a satisfactory effect only after several sessions. Whether one or the other composition will prevail and be declared “state of the art” eventually remains to be seen.

Does the fat dissolving injection help against cellulite?

Yes, the fat dissolving injection also shows good results in the treatment of cellulite (gynoid lipodystrophy). In cellulite treatment, the combination of active ingredients is distributed superficially over the area to be treated at numerous injection points. Results vary from case to case. Individual reports described reductions of 50% and more. In other reports, results fell short of expectations.

As with fighting fat deposits, a healthy diet is often the better way to go when it comes to cellulite. Especially in the case of large-scale occurrence. More fruit, plenty of vegetables, plus a protein-rich diet: this often helps better and more sustainably. Even though cellulite occurs equally in fat and thin women, it is still recommended to consider dieting if you are overweight. Because existing cellulite promotes new cellulite. The tissue is less well supplied with blood and the lymph cannot drain as well. The natural breakdown of fat is thus inhibited, sometimes leading to a steady deterioration of the condition.

Which regions cannot be treated with fat dissolving injection?

The Network Global Health, an association of physicians occupied with the treatment and development of the fat dissolving injection, does advise against treatment in only 3 cases: on the face, the bags under the eyes, and on the legs, the insides of the knees and the ankles. As for the upper face, the use of the fat dissolving injection here actually requires a sense of proportion and special care. The fat pads of the face contribute significantly to a fresh and youthful appearance.

Are there any side effects with the Fat dissolving injection?

The treatment with the fat dissolving injection is reasonably safe. Complications are rare and mostly not severe. However, the treatment can cause significant swelling and an unpleasant burning and itching sensation in the treatment area. This can be particularly unpleasant in the case of a treated double chin.

Side effects, which rarely occur but cannot be ruled out, include persistent redness, permanent lumps and dents due to uneven fat reduction, vegetative disorders such as short-term circulatory weakness, increased sweating and nausea, diarrhoea, irregular menstrual bleeding and allergic reactions. If the blood supply to the tissue is poor, there is also a small risk of necrosis.

In my experience, however, any discomfort disappears 2–3 days after treatment. I strongly advise my patients to refrain from sport and strenuous activity for at least 14 days following the treatment to support the healing process. If the treatment was performed on the abdomen, hips or extremities, tight clothing should be avoided at all costs for a similar period. The risk of necrosis, which exists with the fat dissolving injection, although with a low incidence rate, is reduced even further by these precautions.
